Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while assisting patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their established goals and coach families on safely assisting with exercises. Work as part of a home health team focused on the patient's plan of care and goals. Requires a CA active PT license, one year of experience, and CPR certification.
Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in CA
- Minimum of 1 years of physical therapy experience
- CPR certification
- Knowledge of state and federal home health regulations
- Good clinical judgement
- Strong interpersonal skills
- Dexterity
- Compassion
