Job Summary
Provide home-based physical therapy to patients who have lost mobility, strength, and functionality due to illness or injury. Conduct assessments, document progress, and modify care plans while observing movements. Assist patients with exercises and hands-on therapy to manage pain and increase mobility. Educate patients on their plan of care, established goals, and self-care techniques. Coach families on therapy goals and safely assisting with exercises. Work as part of a home health team including RNs, LPNs, CNAs, and OTs focused on patient outcomes. Maintain a 1:1 therapist-to-patient ratio with flexible assignments and autonomy. Required Qualifications
- Graduate of an accredited Physical Therapist Program
- Active PT license in Kansas
- CPR certification
- Good clinical judgement
- Strong interpersonal skills
