Deliver in-home physical therapy services to adult and geriatric patients by evaluating mobility, strength, and function to develop individualized treatment goals. Maintain accurate and timely home health care documentation per Medicare and agency standards. Required Qualifications
- Active physical therapy (PT) license in good standing
- BLS CPR certification
- Reliable transportation to see patients in their homes
