Provide one-on-one home health PT evaluations, reassessments, and discharge planning; develop individualized treatment plans; deliver skilled physical therapy services in patients’ homes; educate patients and caregivers on exercise programs and safety; coordinate care with physicians and the interdisciplinary team; complete documentation accurately and on time.
Required Qualifications
- Active Washington, DC Physical Therapist license required
- Valid driver’s license and reliable transportation
- Strong communication and documentation skills
- Home health experience preferred
