Job Summary
Complete physical therapy evaluations, reassessments, and discharge visits to help patients regain strength, improve mobility, and restore balance. Develop individualized plans of care based on patient goals while providing skilled services in patients' homes. Educate patients and caregivers on therapeutic exercises, mobility training, fall prevention, and home safety. Monitor progress and modify treatment plans as needed, collaborating with physicians and the interdisciplinary care team. Document all clinical activities accurately using WellSky. This PRN role offers flexible scheduling and one-on-one patient care within established communities across Washington, DC, Maryland, Virginia, and Pennsylvania.
Required Qualifications
- Active Washington, DC Physical Therapist License
- Valid Driver's License
- Reliable Transportation
- Strong Clinical Assessment Skills
- Strong Documentation Skills
