Job Summary
Evaluate patients' functional status, mobility, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve strength, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care. Collaborate with nurses, physicians, and interdisciplinary teams to ensure coordinated recovery. Document care accurately within the EMR and supervise Physical Therapist Assistants per state guidelines. Requires current DPT or Master's degree, active PT license, 1–2 years experience, CPR certification, and valid driver's license. Flexible scheduling and autonomy in a field-based environment.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
