Job Summary
Conduct home mobility assessments evaluating balance, gait, strength, and range of motion to identify environmental barriers. Develop and implement individualized treatment plans for patients recovering from illness, injury, surgery, or managing chronic conditions. Train clients in safe transfers, ambulation, and stair negotiation while educating caregivers on fall prevention and assistive device use. Monitor patient progress, reassess functional status, and modify plans based on clinical findings. Collaborate with physicians and the interdisciplinary care team to support patient goals. Complete timely documentation using the agency's electronic medical record system while maintaining compliance with clinical and regulatory standards.
Required Qualifications
- Active New York State Physical Therapist license

- Experience with adult rehabilitation preferred
- Strong understanding of gait, balance, and functional movement training
- Ability to work independently in a home care setting
- Strong communication and clinical reasoning skills
- Prefer functional, real-world rehabilitation over clinic-based treatment
