Job Summary
Make home visits to clients in designated geographic territories throughout the Hatboro/Horsham area. Perform diagnostic tests and measurements, including mobility, range of joints, transfer status, balance, and safety assessments. Implement integumentary protective interventions to prevent and promote wound repair. Develop and implement individualized care plans featuring manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise the Physical Therapy care plan while participating with nursing in the multidisciplinary care plan. Educate clients and family members in rehabilitative care and activities to promote independent living. Accurately document observations and interventions using a state-of-the-art touch pad tablet.
Required Qualifications
- A current PA Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
