Physical Therapist, PT, Home Health Visits

Job Summary
Conduct home health visits to adult and geriatric clients in the Upper Montgomery County territory. Perform diagnostic tests and measurements for mobility, strength, balance, and safety. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating in multidisciplinary discussions with nursing. Educate clients and family members on rehabilitative care and independent living strategies. Accurately document observations and interventions using a state-of-the-art touch pad tablet. Requires a current PA Physical Therapist license, one year of recent work experience, and graduation from an accredited program. Required Qualifications
- A current PA Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
