Job Summary
Conduct home health visits to adult and geriatric clients across Cumberland, Hoke, and Harnett counties. Perform diagnostic tests for mobility, balance, and strength; implement integumentary protective interventions for wound care; and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while collaborating with nursing staff. Educate clients and family members on rehabilitative activities and accurately document all observations and interventions using a touch pad tablet. Requires a current North Carolina Physical Therapist license and experience with electronic medical records. Full-time or per diem opportunity with a $5,000 sign-on bonus.
Required Qualifications
- A current North Carolina Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently
