Job Summary
Conduct home health visits to adult and geriatric clients in New Castle County, performing diagnostic tests for mobility, strength, balance, and safety assessments. Implement individualized care plans featuring manual therapeutic exercises, gait training, and integumentary protective interventions for wound repair. Continually assess and revise care plans while collaborating with nursing staff, and educate clients and family members on rehabilitative activities. Accurately document all observations and evaluations using a touch pad tablet. Requires a current DE Physical Therapist license, one year of recent experience, and a DPT degree from an accredited program.
Required Qualifications
- A current DE Physical Therapist - PT license.
- A minimum of one year of recent work experience as a Physical Therapist - PT.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
Desired Qualifications
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
