Job Summary
Evaluate patients' functional status, mobility, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve strength and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions to discharge planning. Collaborate with nurses, physicians, and interdisciplinary teams to ensure coordinated care and document all activities accurately within the EMR. Supervise Physical Therapist Assistants in accordance with state guidelines and participate in case conferences. Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
