Physical Therapist (PT) - Home Health - PRN

Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve mobility, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care. Collaborate closely with nurses, physicians, and interdisciplinary teams to ensure coordinated care and accurate EMR documentation. Supervise Physical Therapist Assistants in accordance with state guidelines. This role offers flexible scheduling and autonomy in a field-based environment where you help patients regain independence and quality of life from the comfort of their homes.
