Job Summary
Provide home-health physical therapy to patients in their homes: evaluate functional status, mobility, strength, and rehabilitation potential; develop individualized care plans with physicians; deliver hands-on therapy to improve mobility and independence; educate patients and caregivers on treatment plans; monitor progress, adjust plans, and contribute to discharge planning; supervise PTAs and support staff; document care in the EMR; participate in team meetings and care coordination; requires DPT or Master’s degree and active PT license, CPR, and driving access; 1–2 years of home health experience preferred and autonomy in field-based settings.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
