Job Summary
Home Health Physical Therapist (PRN) delivers patient-centered, hands-on therapy in patients’ homes to improve mobility, strength, balance, and independence. Works autonomously with interprofessional teams to evaluate functional status, develop individualized care plans, educate patients and caregivers, monitor progress, manage transitions of care, and supervise PTAs per state guidelines. Emphasizes building relationships with patients and families, documenting care in EMR, and collaborating with nurses, therapists, and physicians to ensure coordinated care. Requires DPT or Master’s, current PT license, CPR certification, and a valid driver's license with reliable transportation.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
