Physical Therapist, PT - HOME HEALTH PRN

Conduct patient consultations to assess physical condition and symptoms, then diagnose movement dysfunctions and devise effective treatment plans. Educate patients on therapeutic exercises and utilize massage and stimulation techniques to facilitate healing. Assist patients with mobility equipment such as wheelchairs or walkers while maintaining detailed patient records and monitoring progress toward goals. Provide advice on in-home treatment options and exercises to patients and their families. This role supports a diverse range of patients in regaining mobility within a collaborative home health care environment.
