Job Summary
The PRN Home Health Physical Therapist treats patients using mechanical means such as exercise and heat to improve their physical functioning. Responsibilities include providing physical therapy services per physician's prescription, conducting assessments (including OASIS assessments), developing and revising care plans in collaboration with healthcare teams, and reporting patient reactions and changes in conditions to the physician. A physical therapy license in Florida is required along with at least one year of home health care experience preferred. Candidates with diverse backgrounds are encouraged to apply.
Required Qualifications
- Licensure as a physical therapist in the state of Florida
- Minimum of one (1) year experience in home health care preferred.
