Job Summary
Perform comprehensive patient assessments, including OASIS assessments at required intervals, and develop individualized plans of care as patient needs evolve. Deliver skilled physical therapy services and treatments while counseling patients and families on disease processes, self-care techniques, and prevention strategies. Collaborate with the interdisciplinary team to ensure seamless, holistic care in home health visits across Manatee county. Requires an active Florida PT license with at least one year of experience; home health care experience and proficiency with Homecare Homebase EMR are preferred. Required Qualifications
- Active Florida PT license
- Minimum of 1 year experience as a licensed Physical Therapist (PT)
- Willingness to participate in weekend and holiday on-call rotations
Desired Qualifications
- Home health care experience
- Experience using Homecare Homebase (HCHB) EMR
- Strong assessment, critical thinking, and patient education skills
