Job Summary
Conduct direct patient care, evaluate functional needs, and adjust care plans to ensure optimal outcomes for home health patients. Design and implement individualized plans of care based on thorough assessments, while directing support staff to achieve functional goals. Document evaluations, treatment goals, and prescriptions, and consult with specialists as needed. Promote physical therapy in the community and uphold Elara Caring's mission through quality care delivery. This physically demanding role requires lifting up to 50 pounds and potentially working irregular hours including call rotations. Candidates must hold an APTA-approved license, one year of healthcare facility experience, and be willing to travel 50% within the coverage area.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Must be able to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
