Physical Therapist - PT- Home Health - PRN

Job Summary
Provide coordinated physical therapy to clients across ages in home health settings. Plan, implement, and evaluate care plans to restore or maintain health. Conduct clinical assessments, document Plan of Care and progress notes, apply infection control, and plan treatments for neuro-muscular and orthopedic disorders based on referrals. Communicate with clients/families and health-care teams; participate in case conferences; ensure compliance with third-party payer requirements; maintain continuing education and licensure. Requires home visits, travel locally, and flexible scheduling; BS in Physical Therapy and state PT licensure required; valid driver’s license and CPR certification; ability to lift/transfer up to 50 lbs; Prolonged standing/walking and home-based environment readiness.
Required Qualifications
- Two (2) years experience as a PT in a clinical care setting
- Bachelor of Science in Physical Therapy from an APTA approved program
- Current driver’s license in good standing
- Reliable transportation and auto liability insurance
- Current State professional license as a PT in any/all applicable states
- Current CPR
