Job Summary
Physical Therapist (PT) in home health, PRN position. Provides physical therapy services per physician’s written plan of care, assesses functional status, ROM, gait, strength; directs therapy sessions; educates patients and families in therapeutic appliances; supervises PT assistants; contributes to discharge planning; collaborates with care team; maintains documentation and progress summaries; may train in prosthetic devices and coordinate with home health staff.
Required Qualifications
- Degree from a baccalaureate or Master’s program in physical therapy accredited by the American Physical Therapy Association
- Valid and current license to practice physical therapy in state employed
- Two (2) years of clinical experience as a physical therapist; community/home health experience preferred
- Good verbal and written communication and organizational skills
- Current CPR Certification
- Must be a licensed driver with insured automobile
- Capable of performing job functions with or without accommodation
- Provides physical therapy services per physician’s plan of care
- Supervises physical therapy assistants according to policy and state regulations
- Instructs and trains patient/family/caregiver members in therapy procedures when indicated
- Participates in discharge planning and coordination of care
- Maintains communication with other personnel involved in patient care
- Performs other duties as assigned
