Job Summary
Conduct initial and ongoing interdisciplinary assessments of patients, including OASIS evaluations, manual muscle examinations, gait analysis, and range of motion measurements. Develop and evaluate care plans in partnership with physicians, caregivers, and the interdisciplinary team while providing therapeutic treatments and equipment evaluations. Instruct patients and family members on device usage such as wheelchairs, braces, and crutches, and evaluate home environments for safety recommendations. Communicate regularly with healthcare practitioners regarding patient reactions and condition changes, providing education and counseling aligned with individualized needs. Requires Arizona State Board of Physical Therapy licensure, one year of experience, and current CPR, TB, and health clearance.
Required Qualifications
- Education and qualifications commensurate with § 484.115(h) (Condition of participation: Personnel qualifications / Physical Therapist)
- Licensed as a physical therapist pursuant to ARS Title 32, Chapter 19 by the Arizona State Board of Physical Therapy
- Core competency per skills checklist
- At least one year of experience in physical therapy
- Fingerprint card, DMV and criminal background check, as appropriate
- Current CPR, TB and health clearance
- Complete 6 hours of in-service training per year
