Job Summary
Provide home-based physical therapy to patients to improve mobility, strength, and function in their daily living environment; work as part of a home health team and educate patients and families on plans of care, exercises, and self-care; maintain assessments, document progress, and adapt care plans while delivering 1:1 therapy in home health settings. Must have Massachusetts PT license, graduate of an accredited PT program, 1 year of PT experience (ideally in home health), CPR certification, and knowledge of home health regulations. Required Qualifications
- Graduate of an accredited Physical Therapist Program and active PT license in Massachusetts
- Minimum of 1 year of physical therapy experience, ideally in home healthcare
- CPR certification
- Knowledge of state and federal home health regulations
