Physical Therapist (PT) - Home Health - Full Time

Job Summary
Provide autonomous in-home physical therapy to patients: evaluate functional status, mobility, strength, and rehabilitation potential; develop and implement individualized care plans in collaboration with physicians; deliver hands-on therapy to improve mobility and independence; educate patients and caregivers on treatment plans, exercises, and safety; monitor progress, adjust plans, and contribute to discharge planning; collaborate with nurses, therapists, and physicians; document care in EMR; supervise PTAs per state guidelines; support caregivers and staff with guidance and education; licensed PT with DPT or Master's, CPR, and valid driver's license.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Minimum 1–2 years of experience (home health experience preferred)
