Job Summary
Home Health Physical Therapist responsible for delivering patient-centered, autonomous care in patients' homes. Responsibilities include evaluating functional status and mobility, developing individualized care plans with the care team, hands-on therapy to improve mobility and independence, patient and caregiver education, monitoring progress, contributing to discharge planning, and collaborating with an interdisciplinary team. Supervises PTAs per state guidelines and supports caregivers and staff. Requires DPT or Master’s in Physical Therapy, active PT license, CPR, and driving privileges; 1–2 years of home health experience preferred. Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master’s)
- Current PT license in the state of practice
- Minimum 1–2 years of experience (home health experience preferred)
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
