Job Summary
Physical Therapist (PT) in Home Health providing skilled physical therapy services per physician orders and plan of care, including patient assessment, treatment using modalities, and family coaching; specialty programs include Balance and Fall Prevention, Spine Safety, Total Hip and Knee Replacement, and Total Shoulder Replacement; must be licensed in the state of employment, have a valid driver’s license and auto insurance, and be able to drive in all weather to perform home-based visits under supervision of the Branch Director.
Required Qualifications
- Must possess a valid state driver’s license
- Must possess automobile liability insurance
- Must wear seatbelts at all times while driving
- Must have dependable transportation kept in good working condition
- Must be able to drive an automobile in all types of weather conditions
- Must be currently licensed in the state of employment
