Physical Therapist, PT - Home Health - FT

Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential to develop and implement individualized care plans. Provide hands-on therapy to improve mobility, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care with collaboration across the interdisciplinary team. Document patient care accurately within the EMR and supervise Physical Therapist Assistants in accordance with state guidelines. This role offers flexible scheduling and autonomy in a field-based environment within the VitalCaring Orlando Branch, serving Orange and Osceola Counties.
