Job Summary
Evaluate patient functional status, mobility, and rehabilitation potential to develop individualized care plans. Provide hands-on therapy to improve strength, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care and discharge planning. Collaborate with nurses, physicians, and interdisciplinary teams to ensure coordinated treatment and participate in case conferences. Document patient care accurately within the EMR and supervise Physical Therapist Assistants in accordance with state guidelines. Requires current DPT or Master's degree, state PT license, 1–2 years experience, and valid driver's license.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license
- Reliable transportation
- Auto insurance
