Job Summary
Evaluate and assess patients' functional status, mobility, strength, and rehabilitation potential to develop individualized care plans. Deliver hands-on therapy to improve mobility, balance, and independence while educating patients and caregivers on safety strategies. Monitor progress, adjust treatment as needed, and support safe transitions of care and discharge planning. Collaborate with physicians, nurses, and PTAs to ensure coordinated care, document progress in the EMR, and participate in case conferences. Supervise PTAs in accordance with state guidelines and provide guidance to support staff.
Required Qualifications
- Graduate of an accredited Physical Therapy program (DPT or Master's)
- Current PT license in the state of practice
- Minimum 1–2 years of experience
- Strong clinical assessment and critical thinking skills
- Excellent communication and patient education abilities
- Current CPR certification
- Valid driver's license, reliable transportation, and auto insurance
