Physical Therapist (PT) – Home Health & Facility Opportunities

Perform patient evaluations and assessments, develop and implement individualized treatment plans, and monitor patient progress while adjusting care as needed. Educate patients and caregivers on treatment goals and exercises, collaborate with interdisciplinary healthcare teams, and complete required clinical documentation. Patient populations include orthopedic, neurological, and post-surgical rehabilitation, as well as spinal cord injury, brain injury, long-term care, and geriatric residents. Flexible scheduling options are available for PRN, part-time, and full-time roles across home health and facility settings in the Denver Metro Area.
