Physical Therapist (PT) – Home Health & Facility Opportunities

Perform patient evaluations and assessments, then develop and implement individualized treatment plans for orthopedic, neurological, and post-surgical populations. Monitor patient progress and adjust care as needed while educating patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation across home health, skilled nursing, and long-term care settings.
