Physical Therapist (PT) – Home Health & Facility Opportunities

Conduct patient evaluations and assessments, then develop and implement individualized treatment plans for orthopedic, neurological, and post-surgical populations. Monitor patient progress, adjust care as needed, and educate patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation across home health and facility settings, including skilled nursing and long-term care. Flexible scheduling available for PRN, part-time, and full-time roles throughout the Denver Metro Area.
