Physical Therapist (PT) – Home Health & Facility Opportunities

Perform patient evaluations and assessments, develop and implement individualized treatment plans, and monitor patient progress to adjust care as needed. Educate patients and caregivers on treatment goals and exercises while collaborating with interdisciplinary healthcare teams and completing required clinical documentation. Manage assignments across home health and facility-based settings including rehabilitation, skilled nursing, and long-term care for populations such as orthopedic, neurological, and geriatric patients. Weekly pay and flexible scheduling options are available for PRN, part-time, and full-time roles throughout the Denver Metro Area.
