Perform patient evaluations and assessments, then develop and implement individualized treatment plans. Monitor patient progress and adjust care as needed while educating patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation for home health and facility-based assignments. Patient populations include orthopedic, neurological, and post-surgical recovery cases, as well as spinal cord injury, brain injury, geriatric, and long-term care residents.
