Physical Therapist (PT) – Home Health & Facility Opportunities

Perform patient evaluations and assessments to develop and implement individualized treatment plans. Monitor patient progress, adjust care as needed, and educate patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation for orthopedic, neurological, and post-surgical rehabilitation cases. Support home health patients, spinal cord injury survivors, brain injury rehabilitation recipients, long-term care residents, and geriatric patients throughout the Denver Metro Area.
