Job Summary
Perform patient evaluations and assessments, then develop and implement individualized treatment plans for orthopedic, neurological, and post-surgical rehabilitation cases. Monitor patient progress, adjust care as needed, and educate patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation for home health and facility-based assignments across Denver Metro Area settings.
