Physical Therapist (PT) – Home Health & Facility Opportunities

Perform patient evaluations and assessments, then develop and implement individualized treatment plans for orthopedic, neurological, and post-surgical populations. Monitor patient progress, adjust care as needed, and educate patients and caregivers on treatment goals and exercises. Collaborate with interdisciplinary healthcare teams and complete required clinical documentation across home health and facility-based settings including skilled nursing, long-term care, and rehabilitation. Opportunities are available in the Denver Metro Area for PRN, part-time, and full-time schedules.
