Job Summary
Evaluate patient conditions, establish measurable treatment goals, and develop individualized therapy plans using approved techniques. Provide one-on-one care to home health patients while documenting progress, educating families on mobility, and recommending home safety modifications. Coordinate comprehensive care with nursing, social work, and clinical staff, supervising PTAs and HHAs to ensure all services meet HIPAA standards. Requires an active Ohio PT license, minimum one year of experience, current TB test, CPR certification, and valid driver's license. Full-time and part-time roles available in multiple states.
