Job Summary
Evaluate patient conditions, establish measurable treatment goals, and develop individualized therapy plans using approved techniques. Provide one-on-one care by supervising PTAs and HHAs, documenting progress accurately, and educating patients on mobility and adaptive equipment. Assess home environments for safety modifications and coordinate comprehensive care with nursing, social work, and other clinical staff while maintaining HIPAA standards. Full-time and part-time roles available in Ohio with competitive per-visit rates starting at $75.00.
Required Qualifications
- Active Physical Therapist (PT) license in OH
- Valid driver's license
- Reliable transportation
Desired Qualifications
- Minimum 1 year of PT experience
