Physical Therapist (PT) Home Health- Cincinnati

Evaluate patient conditions, establish measurable goals, and develop individualized treatment plans using approved therapy techniques. Deliver one-on-one care by supervising PTAs and HHAs, documenting progress accurately, and educating families on mobility and adaptive equipment. Assess home environments for safety modifications and coordinate comprehensive care with nursing, social work, and clinical staff while maintaining HIPAA standards. Requires an active Ohio PT license, minimum one year experience, current TB test, CPR certification, and valid driver's license. Full-time and part-time roles available with per-visit pay starting at $75, mileage reimbursement, and referral bonuses.
