Job Summary
In-home Physical Therapist (PT) providing adult and geriatric rehab services in Nash and Edgecombe counties, performing home visits, conducting mobility and wound care assessments, developing and implementing individualized care plans (including manual therapy, gait training, balance, and other interventions), educating clients and families, and documenting observations and interventions in the EMR tablet, collaborating with nursing and multidisciplinary teams; full-time field role with per-visit patient care in designated territories.
Required Qualifications
- A current North Carolina Physical Therapist - PT license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
