Job Summary
Conduct home health visits for adult and geriatric clients in Guilford County to provide rehabilitative care and skilled nursing support. Perform diagnostic tests including mobility, strength, balance, and safety assessments, while implementing integumentary protective interventions for wound prevention and repair. Develop and continuously revise individualized care plans involving manual therapeutic exercises, gait training, and balance interventions. Educate clients and family members on activities promoting health, safety, and independent living, and accurately document all observations and evaluations using a touch pad tablet. Requires a current North Carolina Physical Therapist license, one year of recent work experience, and graduation from an accredited PT program.
Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current North Carolina Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
Desired Qualifications
- Solid computer skills
- Prior experience with electronic medical records (EMR)
