Job Summary
Provide expert Physical Therapy services in home health across designated North Carolina counties, conducting client assessments, developing and implementing individualized care plans, and performing therapeutic exercises and gait/balance training. Responsibilities include home visits to adult and geriatric clients, documenting observations and interventions using EMR, educating clients and families, and collaborating with nursing and multidisciplinary teams to promote safe, independent living. Requires NC PT license, ability to work independently, strong interpersonal and organizational skills, and comfort with technology and EMR systems. Required Qualifications
- A current North Carolina Physical Therapist - PT license
- Minimum of one year of recent work experience as a Physical Therapist - PT and home health experience preferred but not required
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) preferred
