Job Summary
Conduct home health visits to adult and geriatric clients in Scotland and Robeson counties to perform diagnostic tests, implement integumentary protective interventions, and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while participating in multidisciplinary care discussions with nursing staff. Educate clients and family members on rehabilitative care and instruct them on activities promoting health, safety, and independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current North Carolina Physical Therapist license, one year of recent work experience, and graduation from an accredited program. Required Qualifications
- One year prior clinical experience as a licensed Physical Therapist
- A current North Carolina Physical Therapist - PT license
- A minimum of one year of recent work experience as a Physical Therapist - PT
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills

- Prior experience with electronic medical records (EMR)
