Job Summary
Conduct home health visits for adult and geriatric clients in Chatham and Northeastern Randolph counties, performing diagnostic tests for mobility, strength, balance, and safety. Implement integumentary protective interventions and develop individualized care plans including manual therapeutic exercises and gait training. Continually assess and revise care plans while educating clients and families on rehabilitative activities. Accurately document all observations and interventions using a touch pad tablet. Requires a current North Carolina Physical Therapist license and one year of recent work experience. Base pay is $68+ per point.
Required Qualifications
- A current North Carolina Physical Therapist - PT license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem solving skills
- Solid computer skills
Desired Qualifications
- A minimum of one year of recent work experience as a Physical Therapist - PT and home health experience
- Prior experience with electronic medical records (EMR)
