Job Summary

Conduct home health visits for adult and geriatric clients in Davidson and Randolph counties, performing diagnostic tests, mobility assessments, and safety evaluations. Implement integumentary protective interventions for wound care and develop individualized care plans including manual therapeutic exercises, gait training, and balance interventions. Continually assess and revise care plans while participating in multidisciplinary discussions with nursing staff. Educate clients and family members on rehabilitative care to promote independent living. Accurately document all observations, interventions, and evaluations using a touch pad tablet. Requires a current North Carolina Physical Therapist license, one year of recent PT experience, and home health experience. Base pay starts at $68 per point.

Required Qualifications

  • Prior home health experience as a licensed Physical Therapist
  • A current North Carolina Physical Therapist - PT license
  • A minimum of one year of recent work experience as a Physical Therapist - PT
  • home health experience
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
  • Ability to work independently
