Physical Therapist (PT) - Home Health

Facilitate rehabilitation for patients with emotional, physical, or mental disabilities by implementing care plans and coordinating with the home healthcare team. Educate patients and family members about health conditions while communicating effectively with caregivers, providers, and office support. Prepare timely, complete, and accurate clinical notes for all assignments. Requires a Physical Therapist licensed in Texas with home health experience, knowledge of regulations, and ability to handle physical workload. Must maintain a good driving record.
