Physical Therapist (PT) - Home Health

Provide skilled physical therapy services in patients' homes under physician orders, delivering interventions to relieve pain, restore or improve function, and maintain optimal performance. Conduct and document OASIS assessments, establish and adjust individualized plans of care, and ensure patient safety while educating patients and families. Supervise PTAs (and, when appropriate, nurse aides); maintain compliant clinical documentation per Medicare/Federal guidelines; collaborate with an interdisciplinary team to optimize patient outcomes. Utilize modalities, therapeutic exercises, manual therapy, and mobility aids; educate families on disease or injury management and decrease re-hospitalizations through targeted visits. Adhere to agency policies and participate in required in-service trainings. Requires active PT license in state of employment and valid driver's license with auto insurance.
