Job Summary
Registered Physical Therapist (PT) in Home Health delivering skilled, intermittent therapy in patients' homes with a focus on reducing documentation and maximizing patient time. Role supports supervision of PT Assistants and Home Health Aide as assigned; involves developing and revising care plans, performing evaluations, administering physician-ordered PT, completing OASIS assessments and progress notes, and educating patients and families on home programs and activities of daily living. Includes a comprehensive benefits package and a $10,000 sign-on bonus for full-time roles.
Required Qualifications
- Graduate from physical therapy curriculum (DPT or equivalent)
- Currently licensed in the state(s) in which practicing
- Two (2) years experience, preferred
