Job Summary
Conduct direct patient care, evaluate functional needs and outcomes, and adjust care plans to ensure optimal results for over 60,000 daily home care patients. Design and implement individualized plans of care based on thorough assessments, while directing support staff to achieve functional goals. Promote Elara Caring's mission and administrative policies, consult with specialists, and maintain accurate documentation of evaluations, treatment goals, and prescriptions. This physically demanding role requires a full range of body motion including lifting and transferring patients up to 50 pounds. Candidates must be willing to travel 50% of the time and report to the Branch Director or Clinical Manager. Compensation ranges from $105,000 to $120,000 annually, potentially including bonuses and on-call pay.
Required Qualifications
- Graduate of an American Physical Therapy Association (APTA) approved physical therapy program
- Current, unrestricted PT license valid for the state of work
- Minimum one (1) year of experience in a healthcare facility
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
- Must be able and willing to travel 50%
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
